Project Title: Contributing Factors of Psychosocial Adjustment of Caregivers of Family Member with Chronic Illnesses/Disabilities

What it is about?
If you are a caregiver who has a family member experiencing chronic illnesses/disabilities, you are invited to participate in this research study in understanding the factors that contribute to the psychological well-being and adjustment of caregivers who have a family member experiencing chronic illnesses/disabilities.

Criteria:
* Adult of age 19 and above
* Having a family member who has experience or is experiencing chronic illnesses/disabilities resulting from injuries and/or illness
* Currently you are the primary caregiver of this individual with the chronic condition
* Primary caregiver means:
* you provide major role in the physical, emotional, familial, and day-to-day support of
your family member

We need your help in:
* We have a packet of survey that we would like you to complete
* It may take no more than 30 minutes of your time. But you can break down the surveys and take a week to complete at your leisure time so that it does not interfere with your daily roles or cause fatigue
* The packet of survey includes basic demographic information, stress, family dynamics and functions, resilience, coping, social support, and psychological well-being

* If you are interested, please help us to complete the survey and mail it back to the primary researchers in the provided self-stamped, self addressed envelope. If you choose to complete the survey online, you can click the link and complete the survey

Rights and Confidentiality:
* Your participation is voluntary
* You have the right to stop anytime if you choose to participate in completing the survey
* Your information is kept confidential. All materials will be kept in the primary researcher’s secured cabinet. Your online survey will only be downloaded by the primary researchers with a password protected computer that is locked in her office.

Examples of Questions:
Some of the questions may be personal. The following are a few examples of the content of some of the items that may consider “sensitive”. Please make your judgment if you feel comfortable proceeding and participating. Should you feel distress while you are proceeding the survey, please refer to the list of resources for you to contact professionals to deal with the distress.
Examples: Are you satisfied in belonging to your family?
How often have you been upset because of something that happened unexpectedly?
Do you feel that you are confident in your ability to care for your family?
I get sudden feelings of panic
I’ve been trying to find comfort in my religion or spiritual beliefs.
